TUTORING SERVICES AGREEMENT FORM

IF UNDER 18 YEARS OF AGE

I, (“Parent”)

I, (“Parent”), parent/guardian for

(Student) agree to retain Tutor the Future, Inc. (“Company”) to provide tutoring and related services for my child.

1. Services: Company is a peer-tutoring company that specializes in Math and Science. Company will arrange to provide tutoring services to the student by a high school senior, college student, or recent college graduate. Company will work closely with the guardian to make sure that the student’s needs are served to the Company’s and its tutor’s abilities. The Company and the tutor will use its best efforts to provide exceptional service to the student, however, by the nature of the service, specific results cannot be guaranteed.

2. Schedule and Cancellations: Company and guardian will create a weekly schedule of tutoring sessions that is reasonably convenient to the guardian and the student and meets the student’s needs. If a session cancellation by the guardian is inevitable, the guardian should notify the Company as soon as reasonably possible, but no later than 8pm of the evening before the session. If a session is not cancelled by 8pm, the guardian will be responsible for a $_10__ non-cancellation fee.

3. Location :Tutoring sessions will take place at the private home of the student or such other convenient location on which Company and guardian may agree from time to time.

4. Fees: For a (1) hour session, pay $

each for (4) students. For multiple sessions during a week, A $ discount from the agreed upon rate will be applied to EACH additional session in the seven-day week. This is subject to change upon mutual agreement between the Company and the Guardian(s). Invoices will be sent out the 1st and 15th of each month. Payment is required within 5 days of sending the invoice in the form of a credit card, debit card, cashier’s check, or money order. All payments must be made payable to Tutor the Future Inc. After the first two (1) hour sessions are completed, at any time you are not satisfied with the tutor, we will reassign another tutor to the student(s) for one session free of charge.

TUTOR AGREEMENT FORM

I,

(“Tutor”), wish to provide tutoring and related services to clients of Tutor the Future, Inc. (“Company”) and therefore, willingly and knowingly agree to enter into this agreement as a condition of providing such services. This relationship is at-will and Tutor may be terminated by the Company with or without notice or reason.

1. Responsibilities/Recommendations Tutor’s responsibilities include, but are not limited to:

Provide an educationally enhanced lesson to each student

Coordinate with in-school teacher on a regular basis – minimum 1x per month (If permitted).

Weekly feedback to parents, as to how student is progressing.

Reporting to Tutor the Future if any unusual behaviors or failing grades occur.

If a session is cancelled by you, a makeup must be set up. The information must be submitted to the office. If you cannot make up a session, the office will assign a substitute for you.

Assigning weekly homework.

Showing up in a timely manner and dressing accordingly.

2. Non Disclosure and Non Use of Confidential Information: Tutor acknowledges that as a result of providing tutoring services for the Company, the Tutor may learn or have access to information such as business plans, customer information, lesson plans, and other information which may be valuable to the Company and its business (“Confidential Information”). Tutor agrees to hold all Confidential Information in its strictest confidence. Tutor will not, at any time, use in any way the Confidential Information for its own account or benefit, and will only use it for the Company’s benefit and to provide services to the Company’s clients.

3. Rights: All trade secrets, inventions, processes, copyrights, trademarks, know-how, improvements, designs, lesson plans, techniques, teaching methods, and other works of authorship, artwork, or other copyrightable or patentable works, created or conceived by the Tutor in performing its services for the Company or learned by the Tutor in performance of services for the Company is deemed will “work for hire” and owned solely by the Company.

4. Non-Solicitation and Non-Circumvention: Tutor acknowledges that the Company’s clients are a valuable asset of the Company, and loss of such clients will result in irreparable harm. Therefore, the Tutor agrees that during the period of time in which he or she is servicing any such client, and for a period two (2) years following termination of such services for each respective client, (a) Tutor will not directly or indirectly solicit, cause another party to solicit any client or potential client of the Company for the purpose of providing tutoring or similar teaching services except for the benefit of the Company and (b) Tutor will not be employed by or provide service (whether directly or via an entity) any person or entity that competes with the Company or services clients within a 40 mile radius of the Company’s office.

5. Expenses: If Company incurs any costs and expenses (including attorneys’ fees) resulting from having to enforce its rights under this Agreement, then the Tutor shall reimburse to Company all such expenses.

6. Payment: In consideration for the Services to be performed by the Contractor, the Company agrees to pay contractor the negotiated rate of

$/per hour. Contractor agrees to submit the Tutoring Timesheet, via mail, email, or text, to the Company in the specified payment period of one month. The Company shall pay Contractor’s fee within 14 days after receiving the timesheet.
